Telescope Health Partners with CommHIT to Expand Virtual Care Access Across Rural Florida

JACKSONVILLE, Fla., (September 16, 2026) – Telescope Health, a physician-led healthcare technology company that improves access to care and health outcomes through innovative clinical services, population health programs, and community-based healthcare solutions, today announced its partnership with CommHIT.

The collaboration supports CommHIT Everywhere, a statewide initiative designed to strengthen healthcare access, connectivity, care coordination, and population health across rural communities. Through the partnership, Telescope Health will provide the clinical operations, technology, and healthcare access infrastructure necessary to support sustainable healthcare delivery within participating rural community pharmacies.

CommHIT recently received a $43.6 million subaward through Florida’s Rural Health Transformation Program (RHTP), administered by the Florida Agency for Health Care Administration (AHCA), to help build and enhance next-generation health infrastructure across rural regions of the state. The initiative is designed to create a more integrated healthcare ecosystem that links community resources, providers, pharmacies, and patients through innovative care delivery models.

The partnership builds upon Telescope Health’s experience creating community-centered access programs, including Healthlink JAX, the innovative healthcare initiative for uninsured Duval residents developed in partnership with the City of Jacksonville. Healthlink JAX serves as a validated model demonstrating how municipalities can improve healthcare access, reduce avoidable healthcare utilization, close care gaps, and connect residents to appropriate care resources.

Through CommHIT Everywhere, participating community pharmacies will serve as trusted healthcare points within rural communities. Residents will have convenient access to virtual clinical consultations, screening services, and coordinated follow-up care. By integrating pharmacy-based access points with virtual care and local healthcare resources, the initiative seeks to create a more connected patient experience while supporting continuity of care across the rural healthcare ecosystem.

Together, CommHIT and Telescope Health are working to strengthen public health infrastructure across rural Florida, helping communities address healthcare access barriers, provider shortages and disparities that have historically limited care options for rural residents. The partnership represents a scalable framework that can support vulnerable populations and bring appropriate care closer to where people live.
