What’s Next?

1. You’ll receive an email from our Virtual Care Management team. Simply click the Activate button in the email to start your registration on our platform:

2. If you already have an account in Telescope Health, your membership should become active immediately and you can use the same email and password you used before. If you can’t remember your password, you can reset at the log in page.

3. Once you’ve created your profile, you can add your family members in the platform by clicking “Add Account” (adding family members requires the use of a computer or laptop using a supported browser (Chrome, Safari, or Firefox):

4. If you are adding a spouse or guardian, be sure to add them as a “co-user”.

5. If you are adding children, choose “dependent”.

6. Once you have created your family in our platform, call our care coordinators to finish the family enrollment process at 904-373-5304.
